Job summary Working within designated clinical areas the post holder will facilitate patient discharge providing a high-quality service in line with Trust standards. They will be able to communicate sensitivity and effectively with a wide range of people including patients, families, colleagues and agencies external to the Trust. The post holder will manage patient flows within a defined area and will be responsible for prioritising work, reviewing working practices to ensure timely and efficient patient access to the Trusts services, delivery of the patient administration pathway from the decision to admit the patient and throughout the inpatient process. Main duties of the job Manage and co-ordinate discharge & patient flows within Chapel Allerton CSU and promote timely decision making to adhere to safe patient placement. To facilitate timely and appropriate patient discharge from Chapel Allerton CSU wards to home, alternative external health or social care provision, or transfer to the appropriate in-patient clinical service.
