This is a key role for a lead in Diabetes dietetics involving both clinical and leadership responsibilities. This role offers a unique opportunity for someone with a passion to improve health outcomes across Hywel Dda and a commitment to develop and learn. Deliver expert level clinical Diabetes Dietetics, managing a complex caseload of patients with Diabetes working across settings (Secondary care, community and primary care) as required.
* Manage a highly complex diabetes caseload, informing multi-disciplinary care and provide expert support and clinical supervision for dietitians / support workers / students Health Board wide.
* Clinically lead the development, implementation, delivery and evaluation of best practice in relation to adult diabetes dietetics Health Board wide.
* Work as an integral member of the adult diabetes multi-professional leadership team, driving excellent nutritional care through evidence based practice.
* Ensure seamless service planning and delivery from prevention through to complex diabetes case management.
* Clinically lead the development and delivery of education and training in relation to diabetes working across Multi profession platforms and multi-disciplinary teams (MDT) to support optimal effective diabetes care.
* Compassionately and effectively line manage members of the diabetes dietetic team. You will be able to find a full job description and person specification attached within the supporting documents.
