Job summary We have a great opportunity for an enthusiastic, highly motivated Advanced Nurse Practitioner to join our friendly and supportive team. Working in collaboration with other members of our multi-disciplinary team you will bring experience in assessment, diagnosis, treatment and management of both acute and chronic conditions. As an independent prescriber you will support our patients with prescribing, issuing and reviewing medications as appropriate. We are part of Community Care PCN comprised of 3 local practices and our large cohort of additional roles staff support and share the delivery of clinical care ensuring our patients see the right clinician for their needs. Main duties of the job An experienced ANP resource for health care professionals and service users, working in collaboration with other members of our multi-disciplinary team. Assess, diagnose, treat, refer or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury. You will hold experience and skills to be able to assess, diagnose, treat and refer patients with a range of chronic diseases and / or specialist support e.g. diabetes, asthma, heart failure, rheumatology. Use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment. An experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice You will demonstrate critical thinking in the clinical decision-making process. Work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
