Come and join our friendly, sociable, experienced teams working within an integrated Health and Social Care community rehabilitation service based in High Weald Lewes & Havens The Joint Community Rehabilitation and Falls Prevention Service is a short-term 7 day service which assesses and treats people within their own homes. Our service aims to prevent unnecessary admissions to hospital or long-term care, enable earlier discharges from hospital and promote independence and quality of life. The ability to travel around the county in a timely fashion is essential. Weekends are covered on a rota basis and at present includes one weekend day approximately every 6 weeks with a day off during the week. Main duties of the job You will be part of a friendly, supportive, professional integrated team which provides a 7-day service and uses an inter-professional model of working. Continuing Professional Development is supported and supervision and training are integral to the working practices of the team. You will key work non-complex clients, provide rehabilitation to people with a variety of conditions, and respond where appropriate to urgent referrals supporting clients to stay in their own home. The post holderwill berequired to work flexibly and visit people in their own home and liaise with clients in acute and intermediate care settings if necessary prior to discharge. The ability to travel around the county in a timely fashion is essential.
