Develop care plans and instigate therapy, based on comprehensive patient focused assessments, relevant guidelines and best available evidence in order to improve health outcomes. Perform capillary blood gas sampling to assess patients suitability for Long term Oxygen Therapy (LTOT), interpreting the results and considering the available prescription options available. To ensure cost effective and appropriate provision of oxygen in the patients home according to national standards. Provide specialist assessment and treatment of patients with COPD and ILD. Support the delivery of the Community Pulmonary Rehabilitation (PR) service. This will include education to groups of patients/carers on condition-specific and sensitive information as well leading exercise programmes. Promote patients to self-manage their condition and encourage behavioural change strategies by identifying where non-pharmacological interventions are appropriate, e.g. smoking cessation, pulmonary rehabilitation, home exercise regimes and improved concordance with medication.
