Role OverviewKey Responsibilities:
Consider whether the findings suggest that the case can be handled “treated” within your skill set or whether the individual requires an onward referral to another member of the MDT team or an external referral.
Where required, post initial assessment, undertake a more detailed assessment within your skill set.
Assist the individual in taking personal responsibility for the longer term and provide them with a plan of action, with scheduled follow ups.
Identify any obstacles to recovery including any psychosocial factors.
Identify whether the individual can remain in their normal duties or whether their role needs to be modified in the short term, also consider whether due to any ongoing condition and or age whether alternative duties needs to be considered.
To be aware of any gender, cultural or religious requirements when undertaking assessment; especially a face to face assessment.
To effectively communicate to the employee the multifactorial issues associated with the current episode of ill health/injury and how they can personally address the risk factors within their locus of control.
To achieve the critical success factors within the various aspects of Functional Health including: reduction in sickness absence; reduction in modified duties; minimisation of any cost of 3rd party suppliers including the costs within the Healthcare Plans; prompt return to normal duties and an overall improvement in the individual’s ability to self-manage and improve their overall health safety, wellbeing and quality of life.
To work seamlessly with the Case Management Team to both take referrals and to provide tasks for them to add a three dimensional approach to the case but specifically to review objective measurements where appropriate.
To maintain knowledge of your specific subject/skill area via regular CPD
To maintain knowledge on ALL aspects of an integrated health risk management approach via regular CPD
To contribute towards Wellbeing information and support for our clients
To hold a case management caseload where required by the business
About the RoleNutrition Case Manager
Fixed Term Contract - up to 12 months Maternity cover
Location: Tewkesbury (hybrid working available)
Salary: £27-35k depending on experience
We are looking for someone who is passionate about Nutrition to join our Corporate Health division on a fixed term contract basis as maternity cover. The Nutrition Case Managers support delivery of Nutrition services across all business service channels through consultations, training and coaching as part of our integrated health and wellbeing model. This is a varied role which would suit an organised and problem-solving clinician who understands how good nutrition can contribute to good overall health and wellbeing inside and outside the workplace.

Essential SkillsSkills and Qualifications:
Degree level qualification(s) in subject matter or equivalent
Demonstrates excellent understanding of nutrition and keep qualifications current
Able to gather and understand business requirements using appropriate tools and techniques and links this back to subject discipline
Experience in a Case Manager or Functional Health role and able to work as part of a multi-disciplinary team.
Proven ability in delivering against an integrated health model
Currently recognised by professional bodies e.g. HCPC, CSP, Reps, BANT, BASRaT
IT proficient - including with MS Office packages e.g Word and Powerpoint.
This role would suit a Case Manager, Nutritionist, Nutritional Advisor or similar
