An exciting opportunity has arisen to join the Specialist Dementia and Frailty Team team within West Essex. We are looking to recruit a motivated, hardworking team leader who has excellent communication skills with a passion for driving service improvement and working collaboratively with other health care providers and third sector agencies. The successful candidate will be responsible for leading the service in Uttlesford in the delivery of a high level service providing a proactive assessment, diagnostic, monitoring and management of patients with dementia of any age and people over the age of 75 years with complex Mental Health needs who have an age related frailty. As part of the national agenda for transformation, the service will focus upon admission avoidance to both general hospital and a mental health inpatient setting by integrated working with the community teams within the local Integrated Neighbourhood Teams in West Essex.
Provide leadership of the Multi Disciplinary Team to deliver the service
Responsible for the overall delivery of a range of evidence based interventions to service users based on identified holistic needs, review the effectiveness of implemented care plans and packages and discharge service users back to primary care / other providers in a timely manner.
Develop robust inter-agency relationships with key stakeholders supporting development of the integrated care pathways and locality working.
Responsible for the coordination and management of the team in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high quality community mental health services.
Responsible for general service management including
• Appraisals
• Supervisions
• Complaints/investigation management
• Management of sickness absence/disciplinary/capability and other workforce procedures and policies.
Support others in their learning and development in relation to the needs of patients with dementia and complex mental health needs alongside age related frailty
To ensure the service works in line with CPA/Social carer/risk management/ safeguarding and other relevant policies.
