Job summary
A rare opportunity has become available to join the Dementia Rapid Response Team covering the High Peak and North Dales area of Derbyshire as a specialist support worker.
The Dementia Rapid Response team has been developed to support people to live in their own homes and to live well with dementia.
The team currently consists of a group of highly skilled individuals who provide rapid, evidence-based intervention to people who are experiencing an exacerbation of their illness and require intense input in order to prevent breakdown of care or hospital admission.
The service runs 365 days a year, Monday-Friday, 8am-8pm, weekends and bank holidays successful candidates will be expected to support the registered staff in delivering specialised, rapid and intensive input and treatment.
The successful candidate will be expected to cover a large geographical area so access to their own car is essential.
Main duties of the job
The main duties of the job role are:
1. To support service users in their own homes and / or in other settingsas agreed, and to undertake tasks in support of service user's careplans.
2. To encourage service users to participate in relevant activities through the development of therapeutic relationships.
3. To support and assist in practical activities outlined in the care plan
4. To maintain effective record keeping
The successful candidate will need to display:
5. Exceptional communication skills
6. Have a caring/compassionate nature
7. Have knowledge and understanding around Dementia Care and Services in the local area.
8. Be able to time manage and react quickly to high risk developing situations with support from qualified staff.
