Main duties of the job The successful candidate will be responsible for the specialised assessment and management of patients with cardiovascular disease (CVD). As an integral member of the multi-disciplinary Cardiovascular Rehabilitation (CVR) team, the post holder will contribute flexibly to the delivery of exceptional care for patients and their families across both primary and secondary care settings. This role encompasses inpatient ward visits, conducting face-to-face clinical assessments, providing home visits for patients, supporting the implementation of CVR classes, and overseeing the ongoing case management of patients. Additionally, the post holder will participate in service development initiatives and engage in research activities to enhance the quality of care provided.
