An exciting opportunity has arisen for an enthusiastic and skilled support, time and recovery worker who wants to be part of an exciting, developing service to provide rehabilitation to adults with mental health needs (primarily psychosis) accessing our residential rehab unit and community rehabilitation services.
This 24-hour community-based service offers supported living for those moving forward with their recovery journey. The successful applicant will need to work to a rota, which will include some evenings, weekends, and night shifts. Due to the nature of the service, lone working will be required.
This is a part-time post of 30 hours per week that will involve providing support and therapeutic time alongside promoting independence to re-establish the person's place in the community.
The post holder will be given a full induction, regular supervision, and personal development reviews with opportunities to work in other areas of the rehabilitation service to gain experience and development. The service follows the philosophy that all contributions matter, and as such, the post holder will have opportunities to attend the weekly multi-disciplinary meetings offering further learning opportunities.
Main Duties of the Job
The successful applicant will provide support; give time to an allocated group of service users with more complex needs and thus promote their recovery. You will work with our 8 residents within our rehabilitation unit at Hilda Johnson House, as well as within our local community. You will also support individuals in their onward journey into the community. You will need to demonstrate enthusiasm and innovation with an appreciation and understanding of the concept of person-centred recovery. Experience of working in mental health is desirable. The team has an extended range of multi-disciplinary members including consultants, senior nurses, psychologists, social workers, support time and recovery workers, healthcare support workers, and occupational therapists.

Job Responsibilities
1. Responsible for providing the link into the care coordination process for an allocated number of individual service users. This will include attending and actively participating in training sessions, team/care plan review meetings, and supervision as appropriate.
2. To ensure that service users engage effectively with the agreed Care Plan and access appropriate services provided on a regular and consistent basis.
3. Positively promote the independent living of service users within their community.
4. Develop a rapport based upon companionship and friendship but within appropriate and transparent boundaries.
5. Provide regular and practical support to service users and their carers in developing and managing dignity and independence.
6. Provide support with daily living; living ordinary lives.
7. Help service users gain access to resources to include benefits and welfare rights.
8. Provide information on health promotion.
9. Help to identify early signs of relapse by monitoring the service users' progress, level of functioning, and mental state and alert the appropriate staff involved in their care.
10. Report regularly to the Unit Manager and Care Coordinators as appropriate key worker(s).
11. To maintain adequate records as required by existing procedures, entering appropriate details on the service users' case notes as necessary.
12. To ensure the service user understands and has a clear pathway of care across sector/agency boundaries with key contact points/named individuals.
13. Participate in the planning of protocols to develop the service to the clients.
14. Participate in the review of the service provided to clients overall by STR workers and advise any changes to the Unit Manager/Care Coordinator.
Person Specification
Qualifications
* NVQ Level 2 or Level 3 or commitment to train towards achieving it.
Experience
* Experience of working with people with mental health needs, providing support to individuals and carers.
