Job summary We are seeking a motivated and experienced physiotherapist to join the established leadership team in the department of medicine for older people (DMOP) & Acute Medical Unit (AMU). The post holder will have significant knowledge and skills within these areas gained through a broad range of experiences, and will be able to provide leadership and clinical expertise. The post holderwill predominantly be based in the DMOP team, but will need to ensure that both teams are efficiently managed and that the department is effectively contributing towards both internal and external objectives alongside national measures. Working under the direction of the Therapy leads for DMOP & AMU, the post holder will be responsible for ensuring that an effective and evidence based service is delivered to those under our care. The post-holder will autonomously assess and treat their own specialist caseload of patients, defining and implementing programmes and group exercise classes. They will liaise with departmental leads to ensure collaborative working in all areas both within the defined teams and across service boundaries. The post holder will lead multidisciplinary team projects, co-coordinating discipline specific complex audit and research projects. The post holder will also be required to work with other senior clinicians to ensure that the service is adequately coordinated to meet the needs of the organisation & local health economy. Main duties of the job Establish highly effective and efficient communication networks with patients, carers and other health workers and internal/external agencies throughout the various departments within the hospital and local community The post holder will work independently and organise his/ her own work/caseload and be accountable for own professional actions To undertake comprehensive, highly specialized assessment and re-assessment of patients, using advanced specialist clinical reasoning skills, investigative and assessment techniques (e.g. assessment of altered muscle tone and x-ray / CT / MRI interpretation) to provide accurate diagnosis and develop an individualised treatment protocol. To be the lead clinician within the MDT responsible for discharge planning of Patient's with complex needs, ensuring safe discharge back into the community and referrals onto appropriate community services To provide opportunity in supervision sessions for the use of critical thinking, reflect on an analysis of case management and personal development To be responsible for the monitoring of staff sickness, leave and working hours in line with departmental guidelines and Trust policy Participate in the operational planning, evaluation and audit of practice, clinical pathways and protocols within area of practice, leading on delegated projects within the therapy team and area of work About us London North West University Healthcare NHS Trust (LNWH) cares for the people of Brent, Ealing, Harrow and beyond.
