Job summary We have a new and exciting opportunity within Swansea Bay District Nursing for the right candidate. We are looking for an experienced and enthusiastic HCSW with a Level 4 vocational qualification in a relevant Health Care Associated subject/ Certificate in HCSW Education. We have a Part TimeBand 4 Assistant Nursing Practitioner position available within Swansea Bay Health Board. The successful candidates will have access to ongoing support and a structured, robust induction and training package in order to develop into role. Applicants to have excellent communication skills and have a flexible and adaptable approach to changing work patterns. Hours of work are between 08:00-22:00 over a 7 day service. Main duties of the job The post holder will be required to deliver high quality, compassionate, evidence-based care to a group of patients following the initial assessment by the Registered Nurse/Practitioner utilising person-centred approaches which promote health and independence. The post holder will carry out a range of clinical skills appropriate to their area of work, in line with their scope of practice and the all-Wales Band 4 Assistant Practitioner (Nursing) Governance Framework. This role involves working within the community, delivering high quality care to a vast range of patients within their own home setting. There would be an expected allocation of work to be complete each shift, this includes visiting patients as a lone worker. The post will also involve transporting equipment such as medical stock and equipment to patients and to health board sites. Therefore, the ability to travel independently between sites in a timely manner is essential.
