We are looking for a Community Nursing Care Co-ordinator. As the Care Coordinator, you will have the overall responsibility for the coordination of the regular care coordination meetings and the smooth running of integrated care within the team setting.
The key role of the care co-ordinator will be to schedule all meetings across all the general practices in their cluster, to manage the meeting agenda items, ensuring all new referrals are identified and information circulated to team members in advance of the meeting. They will update the service user records contemporaneously on the identified IT systems.
Main duties of the job
1. Coordinate and manage the administrative functions for the integrated care meetings. This will include organising and adjusting meetings and tasks on behalf of several stakeholders.
2. Be the point of liaison for service users receiving care coordination and interface with all professionals involved in service users care including primary, secondary, South Central Ambulance Service, community services, carers and other relevant groups.
3. Coordinate the handover with other teams to facilitate safe and effective transition of care between services to provide seamless support for patients, their families and carers.
4. Liaise with medical and nursing personnel in primary, secondary and specialist centres about patients and service provision related matters including statutory and voluntary providers of care.
5. Liaise and communicate with referrers, patients, relatives, carers, and other health and social care professionals to support and coordinate care.
6. Responsibility for the co-ordination of the regular care co-ordination meetings and the smooth running of integrated care within the team setting.
7. Coordinate the necessary assessments, appointments or investigations to facilitate the provision of timely care and treatment.
8. To manage and prioritise workload daily and deal with the competing demands of the integrated care team.

Person Specification
Education/Qualifications/Training
* Good general education including GCSE English and Maths (or equivalent).
* NVQ4 or NVQ3 or recognised certificate in office administration or equivalent experience.
* Evidence of continued role development.
* European Computer Driving Licence (ECDL) and/or competence in the use of Microsoft Word, Excel, Powerpoint, Access and Outlook.
Previous Experience
* Experience of providing confidential administrative support to a complex and varied service.

* Experience in the use of data management.
* Experience of providing administrative support in a health or social care environment at Band 3 (or equivalent).
