Main duties of the job The Role As a Band 5 Assistant Commissioning Nurse in NHS Continuing Healthcare, you'll play a key role in reviewing eligibility for the NHS Continuing Healthcare and NHS Funded Nursing Care in a variety of settings including patients own home, care home or other care facility. You'll help ensure that people receive the right support, whether through NHS Continuing Healthcare (CHC), Funded Nursing Care, or jointly funded care packages. You'll work closely with patients, their families / representatives, and health and social care professionals to ensure that care for eligible people is delivered safely and effectively. Your expertise will help shape care plans, influence quality of care, and safeguard those who are most vulnerable. What You'll Be Doing Triaging new referrals to the service Reviewing eligibility for NHS Continuing Healthcare and Funded Nursing Care in a range of settings (homes, care facilities, etc.); Supporting patients, carers, and professionals with expert advice and guidance; Ensuring that care plans meet the needs of patients in a safe and person-centred way; Monitoring and improving the quality of care provided; Working alongside local healthcare teams, NHS providers, and social care colleagues to ensure the best possible outcomes for our communities.
