Main duties of the job The successful candidate will be able contribute to the assessment, implementation and delivery of high-quality patient care within a professional team. Undertake agreed activities as delegated by a registered nurse, with patients / service users under thedirect /In-direct supervision of the qualified staff. Contributes to the assessment, implementation and delivery of individualised evidence based care. To maintain accurate records in the patient's own home or electronically, record new problems as theyare identified and report all actions to nurse in charge/case manager. The post holder is required to take personal responsibility to incorporate the 6 C vision of care into theircare delivery. Provide feedback of current patient / service user status to appropriate members of the-professionalteam.
