Job summary The Children's Community Nursing Team in York have an exciting opportunity for a Children's Nurse with a minimum of 2 years experience to join the team. We are a friendly, supportive team of experienced and skilled Children's Community Nurses that cover the York and Selby area. This full time Band 5 post will provide clinical care and support to children, young people (aged 0-19yr) and their families, with a wide range of acute and long term health needs in a community setting. You will need to be able to work autonomously as well as part of a team. Excellent communication skills, adaptability and a drive to provide high quality care which empowers children and families is essential for the role. Care will be provided manly through home visits, school or nursery visits. However, the role also involves working in collaboration with the acute areas, Consultants, Specialist nurses and Special school nurses, Tertiary centres, Martin House and Primary care. The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required. Main duties of the job As a Children's Community nurse, you will provide advice, support and nursing interventions to children and young people with both acute short term health needs and children with complex long term conditions within their home and other community settings. You will manage a caseload of children and will play a key role in the assessment of patient care needs and the development implementation and evaluation of programmes of care. Working in partnership with the children, young people and families, as well as carers and other health, education and social care colleagues. Experience of NG and gastrostomy care, Tracheostomy care, IV's and central lines, wound care, complex needs would be beneficial. We provide the shared care for the regional oncology centre at the Leeds General Infirmary. We also offer short term support for medical and surgical patients to facilitate discharge and reduce ward attendance and readmission. The post holder will be expected to work flexibly. The service currently provides Monday to Friday cover from 8am until 6pm. As a team we offer on-call support for end of life care. The post holder will need to be a car driver and be expected to work independently around the region.
