Are you an experienced, registered Mental Health nurse looking for a new community role with a Mon-Fri, 9-5pm working pattern? We’re looking for someone with specialist knowledge of the care of older people to join us in the CMHT (elderly) where we provide mental health care to older people with mental health conditions and co-existing physical health concerns.
You’ll need to be experienced in working in a Community Mental Health Team providing care and treatment using appropriate pathways including the Dementia Pathway in line with NICE guidelines.
You will need to have excellent communication and people skills and ideally, you’ll have a good understanding of Mental Health Framework and legislation, the Mental Health Act & the Mental Capacity Act as well as Clinical Risk policies and procedures.

The role is based at Nicholson House, Maidenhead and your time in this role will be spent visiting clients in their homes in the local community so you will need a car to get to appointments. You will be required to come to the office occasionally for team meetings, training or for other service-related reasons as necessary, although currently we conduct most team meetings virtually this may change in future.
Main duties of the job
1. Undertaking initial, comprehensive assessments in a range of settings including patient’s own home, taking in to account the person’s physical, psychological, social, and spiritual needs.
2. Developing an agreed care plan with the service user and where appropriate include the view of the families and carers.
3. To monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team by reporting in review or at CPA meetings.
4. Managing your own caseload, working collaboratively with other disciplines from both statutory and voluntary sectors.
5. Responding to acute psychiatric problems and able to take new referrals.
6. To have knowledge of a variety of therapeutic interventions short term problem solving, crisis intervention and basic CBT techniques.
7. Participating actively in Service Development contributing to projects/plans aimed at improving and maintaining and quality of care.
