Job summary
West View Surgery has a full time (or part time) vacancy for the role of Care Navigator.
This is an essential front-line role at the practice, where you will play a vital role in joining our team where you will be speaking to patients and other stakeholders on the telephone and in person, managing patient referrals, processing clinical documentation amongst other essential tasks. The role of the Care Navigator is varied and challenging.
Main duties of the job
a.Dealwith all general enquiries
b.Providean effective and efficient reception service to patients and all other visitorsto the practice.
c.Maintainingand monitoring the practice appointment system
d.Processpersonal, telephone and e-requests for appointments
e.Answerincoming phone calls, transferring calls or dealing with the callers requestappropriately
f.Signpostpatients to the correct service
g.Initiatingcontact with and responding to, requests from patients, team members andexternal agencies
h.Dataentry of patient registrations and relevant patient information asrequired
i.Inputdata into the patients healthcare records as necessary
j.Ensuringall letters, reports, results etc are scanned and filed promptly, ensuring that all recent correspondence is availablewhen patients are seen.
k.Creatingclinical referrals to the District Nurse team, Community Matron and Secondary Care via the e-referral system
l.Manageall queries as necessary in an efficient manner
m.Carryout system searches as requested
n.Summarisenew patient medical records
o.Processpatient deductions via the PCSE portal
p.Maintaina clean, tidy, effective working area at all times
q.Monitorand maintain the reception area and notice boards
r.Supportall clinical staff with general tasks as requested
s.Requestingpatient investigations via the ICE system.

Thefollowing are the core responsibilities of the Care Navigator. There may be onoccasion, a requirement to carry out other tasks; this will be dependent uponfactors such as workload and staffing levels:
a. Deal with all general enquiries

d. Process personal, telephone and e-requests for appointments
e. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
f. Signpost patients to the correct service
g. Initiating contact with and responding to, requests from patients, team members and external agencies
h. Data entry of patient registrations and relevant patient information as required
i. Input data into the patients healthcare records as necessary
j. Ensuring all letters, reports, results etc are scanned and filed promptly, ensuring that all recent correspondence is available when patients are seen.

1. * Experience of working in a GP surgery
2. * Experience of administrative duties
3. * Experience of using EMIS Web
4. * Experience of using eRS
5. * Experience of working in a health care setting.
6. * Excellent communication skills (written and oral)
