Fleetwood Surgery - Practice Nurse - 15-32.5 hours per week (primary care experience preferred)

There are several opportunities available, and we are able to offer hours between 15-32.5 hours per week, competitive salary based on experience, and are really keen to meet colleagues who have experience in particular with Diabetes and Respiratory Management.
Main duties of the job
Our Practice Nurse role would be expected to undertake a range of nursing assessments and provide appropriate care or treatment in conjunction with our clinical team and Practice Policies.
Ideally, you'd be able to support and deliver a high level of quality care and service based on the needs of the patient population, which would include treatment, management plans, screening, preventative care, and patient education. In addition to this, you would also be expected to support the Practice in meeting its contractual obligations and delivery of enhanced service specifications.

Fleetwood Surgery is an innovative and forward-thinking Practice led by our partnership of 3 GPs. We serve and cover a local population of approximately 6,500 patients and are looking to recruit and welcome Practice Nurses to expand and enhance our team.
We are a friendly, professional, and caring team who are passionate about providing safe and effective care. Our excellent multi-skilled Clinical Team comprises of 4 GPs (Partner & Salaried), Pharmacists, Pharmacy technicians, Practice Nurse & HCA's.
Our team also includes a Practice Manager, Assistant Practice Manager, Administrators, Secretaries, and our reception team who work in a Patient Advisor role providing enhanced signposting and ensuring patients have access to the appropriate clinicians or services available to them.
Supported by our excellent Clinical and non-Clinical teams, Fleetwood Surgery is a consistently high performer in relation to its contractual obligations including QOF and Quality Contract services.
An established Training Practice, we pride ourselves in our commitment to teaching and support of a variety of clinical educational placements.
Job responsibilities
1. Assess, plan, develop, implement, and evaluate programmes to promote health and well-being.
2. Implement and evaluate individual treatment plans for patients with a known long-term condition.
3. Identify and manage as appropriate treatment plans for patients at risk of developing long-term conditions.
4. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care.
5. Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care.
6. Deliver opportunistic health promotion using opportunities such as new patient health checks.
7. Provide information and advice on prescribed or over-the-counter medication on regimes, side effects, and interactions, as able to.
8. Assess and care for patients presenting with uncomplicated wounds.
9. Support and advise women requesting information relating to family planning needs and general health.
10. Support and manage health needs of women presenting for cervical cytology consultations.
11. Recognise, assess, and refer patients presenting with mental health needs in accordance with the National Framework for Mental Health.
12. Implement and participate in vaccination and immunisation programmes for both adults and children.
13. Advise, support, and where appropriate, administer vaccinations for patients travelling abroad.
14. Promote and deliver evidence-based care for patients presenting with aural conditions.
15. Assist senior partners in providing minor-surgery sessions.

Person Specification
Qualifications
* Diploma in an area of or in Long Term Chronic Disease Management (Diabetes, COPD, Asthma etc).
* Membership of professional body.
* Clinical supervision training and experience.
* Independent/supplementary nurse prescribing qualification.
Experience
* Minimum 3 years post-registration experience.
* At least 2 years recent primary and community nursing experience.
* Experience of nurse-led management of long-term conditions.
* Experience of implementing protocols and clinical guidelines.
* Experience of quality initiatives such as clinical benchmarking.
* Practice experience of working with others.
* Experience of using own initiative.
* Working as part of a team.
* Knowledge of Primary Care Contract Requirements and application of service delivery to meet targets e.g., GMS Contract, QOF, Enhanced Services.
