Description Exciting Clinical Leadership opportunity Well respected NFP aged care provider with Salary Packaging Full-Time (6-month contract) Doncaster location-work close to home About Us We are a highly regarded not for profit aged care organisation offering residential, community and in-home services We are looking for an experienced Clinical Care Coordinator (CCC) to join our team on a short-term 6-month contract. The CCC role is Monday to Friday, full time – flexible with hours. MannaCare Cassia House is an NFP aged care facility with capacity for 90 beds, including a Memory Support Unit. We have experienced and long-term staff, easily accessible resources due to our location, and supportive clinical teams on-site daily to support the clinical care of our residents. Reporting directly to the Operations Manager of the home who is also a RN, clinical governance is key to ensuring you are an appropriate candidate for this position. There may be an opportunity after the end date of the contract to continue working in either CCC or RN role within the organisation. Key duties include: Oversee the day-to-day clinical governance of the facility Ensure the provision of quality care services meets the residents' needs Ensure services are delivered in accordance with the Aged Care Accreditation Standards Lead and mentor the clinical team Staff performance reviews and management of staff Reactive clinical education delivery Monthly trending and data analysis Support AN-ACC documentation to maximise the funding About You Current qualifications as a Registered Nurse with AHPRA Experience in a clinical management role in an aged care setting of more than 2 years Previous experience in overseeing a Memory Support Unit as the CCC Excellent working knowledge of the Accreditation Standards Excellent knowledge of SIRS, trending and data analysis, and restrictive practices.
