If you're someone who is innovative, proactive, and passionate about providing high-quality holistic care, then don't miss your opportunity to apply for these exciting roles. and be part of this amazing transformation Main duties of the job Primary care mental health (PCMH) work closely with our First Contact Practitioners for mental health in the GP surgeries referring into the primary care mental health team, where we have a clinical care navigator and nurses offering triage, assessment and intervention. We have daily interface meetings with talking therapies, community mental health teams, early intervention, home treatment, psychology and occupational therapy to support the 'no wrong door' approach to referrals. Referrals are discharged from the primary care team or may be referred into the community mental health teams who continuing their development with new roles offering assertive outreach and dual diagnosis advice and guidance taking into account the particular needs of our four places (Dudley, Sandwell, Walsall and Wolverhampton). Our new model will focus on supporting people living in their communities with mental health challenges. The Community Mental Health Team (CMHT) will have responsibility for managing a caseload of service users with mental health needs who have been referred to the team via a number of access points. As a further extension of this role you will also be involved in crisis management as part of the duty system and you will undertake the role of the duty nurse on a rostered basis. You will work from our community base facility, via phone/agile working or within the service user's home.
