Job Summary Provide leadership for the personal care and Hospice at Home service. Be the first point of contact for all families / carers / professionals phoning and receiving visits from the service. Hold a caseload and be responsible for the initial assessment and review of the patients in this caseload. Line manage a team of health care assistants and band 4 coordinators with support from the deputy community services manager (DCSM). The post holder must work in accordance with the Nursing and Midwifery Council (NMC) code of professional conduct and Wirral Hospice St Johns Core Values and Policies and Procedures. Work flexibly over both personal care service, hospice at home and other community services as required. Role and Responsibilities Clinical and Professional Support and work closely with the senior nurses and other members of the team to plan care, direct and manage the service so the strategic vision and operational objectives for the delivery of specialist palliative to patients and their families within the community are achieved. Assist in the delivery of clinical care and associated activities in line with the appropriate standards and guidelines. Liaise and work collaboratively with all staff within Wirral Hospice St. Johns, NHS staff, Continuing Health Care, Intermediate discharge team and other Health Care Professionals within the locality and other to promote positive relationships and ensure sufficient effective running of the service. Accept referrals and gather information to ensure the patient and those who are supporting them receive the service they require. Complete holistic nursing assessments via telephone and when required in the patients home. Identifying potential risks within the home environment to ensure patient and staff safety. Use enhanced communication skills to engage with patients, carers, and relatives, including those with language and cultural differences. Review patients over the phone or in their homes as per service criteria. Triage telephone calls and signpost accordingly. Have a knowledge of common symptoms and anticipate changes at the end of life. Support the timely co-ordination of care calls, to ensure smooth efficient running of the service. Lead the team handovers for HCAs in both personal care and support sit services, ensuring they have all the relevant information required. Provide support and reflection as part of this process. Manage and maintain SystmOne records and ensure all details are correct and up to date. Participate in setting standards and the audit of patient and family care. Be responsible for identifying and acting on any risk of safeguarding to patients and their families in accordance with Wirral Hospice St. Johns safeguarding of vulnerable adults and the safeguarding of children policies and procedures. Personal / Professional Development Supervision. Engage in regular supervision one to ones with DCSM and clinical supervision reflective practice opportunities. To support the DCSM in completing appraisals and midyear reviews for support staff. Registration. To maintain professional registration with regulatory body. To abide by the Code of Conduct / Practice for registered regulatory body. Maintaining updated practice. Completing Hospice Mandatory Training within required time frames through required medium. Identifying and attending training as identified in annual appraisal process. Attendance of locality / network meetings in agreement with CSM Awareness of regional and national policy relevant to area of work and underpinning approaches. Demonstrate continued development of own critical analytical skills Knowledge of evidence based practice through literature reviews, study days and courses as relevant to identified CPD. Taking part in reflective practice and in-house training opportunities.
