Community Mental Health Nurse – band 6 We have an exciting opportunity to offer the right person to be a part of our committed and enthusiastic team working with people who have complex needs relating to their diagnosis of dementia and mental health illness. Our service is in the process of moving away from a more traditional model to adopt the follow me care principles advocated in NHS England’s new Community Mental Healthcare Strategy. We are confident that this approach will help us to meet the growing demands for services whilst continuing to provide good outcomes for people. This post will sit within our multidisciplinary community dementia team and will work as a lead practitioner to deliver interventions to those people who have more complex or intensive needs. We aim to help people to remain safe and well at home and able to maintain the relationships that are important to them. The service has an intensive support element within it that offers extended hours and weekend interventions to people who are seen by the team. They will demonstrate an understanding of clinical interventions relevant the care group to include education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students. This post offers the Trust's £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).
