We are looking for an enthusiastic, motivated and committed pharmacy technician to lead on the delivery of a high-quality clinical pharmacy service to the Inpatient wards and Community services in the West Kent locality. Working as part of our medicines management senior management team, you will lead and manage pharmacy technicians in the locality to ensure they are supported to work both in the inpatient and community settings.
The post holder needs to have excellent communication skills, and a desire to work on the front line in mental health with patients, carers and healthcare professionals alike as part of our innovative mental health pharmacy team. We are passionate and dedicated to ensure medicines safety is at the heart of patient care.

Main duties of the job
Take on operational responsibility for managing pharmacy technicians in their locality to ensure safe and effective delivery of medicines management services within the inpatient and community services.
Working alongside the lead clinical pharmacists, key aspects of this clinical post will include managing pharmacy technicians to provide support and advice on medicines management to the inpatient and community services. This will include focusing on medicines reconciliation on admission, discharge facilitation, patient counselling, medicines supply and ensuring that pharmacy staff in the community teams are competent and confident in delivering pharmacy services in all clozapine and depot clinics within their locality.
You will have a particular role in implementing and monitoring the inpatient and community clinical pharmacy standards and working closely with the Chief, Deputy Chief Pharmacist and Lead Clinical Pharmacists to identify service gaps and areas for improvement.
You will also be involved in providing training and undertaking and developing audits and projects to evaluate and improve services alongside pharmacy and trust priorities and objectives.
Job responsibilities
The roles and duties of this post include:
* Managing pharmacy technicians in the locality to ensure medicines management processes are to a high standard across all sites.
* To provide a comprehensive medicines management service to the mental health unit, including ward pharmacy visits, multidisciplinary team meetings and community and crisis team support.
* Promote seamless and integrated care for patients with mental health conditions, supporting the transition of patients across care settings so they receive the required level of care to support them in taking their medication safely in the most appropriate care setting.
* Support discharge planning to provide a smooth and safe transition of care. Ensure seamless pharmaceutical care across the KMPT teams where the post is based.
* Be responsible for relevant teaching and educational programmes for the staff in the inpatient, community services and for health care professionals working in primary care.
* Discussing medication with patients and carers and developing a variety of approaches to support informed care.
* Educating colleagues about medicines management and medicines optimisation.

Qualification
Essential
* NVQ 3 or equivalent
* Registered with GPhC
* Medications management knowledge including appropriate policies and legislation acquired through formal education, plus specialist training and experience to the equivalent of degree level.
* Post graduate leadership/management qualification
Desirable
* Accredited Medicine management qualification
* Accredited accuracy checking qualification
* Experience in using the Point of Care Haematology (PocHi) machine
* Member of the College of Mental Health Pharmacy

* 3 years' experience of working in Mental Health services particularly in the inpatient and community teams
* Working in various areas within the Trust and across organizational boundaries, linking primary and secondary care
