Position Overview:
We are seeking an experienced and dynamic GP Pharmacist Independent Prescriber to join our client's busy GP practice on a full-time, locum basis. The ideal candidate will have significant experience working within a GP practice setting, a strong working knowledge of the EMIS system, and hold independent prescribing qualifications.
As a key member of the clinical team, you will provide pharmaceutical expertise and play a vital role in the safe, effective, and responsible management of patient care. You will work closely with GPs, nurses, and other healthcare professionals to ensure optimal outcomes for patients, supporting a wide range of clinical services.
Key Responsibilities:
* Provide expert advice and support to patients regarding the use of medications, including the safe prescribing and monitoring of treatment.
* Manage repeat prescriptions, formulary adherence, and medication reviews in line with practice protocols.
* Work closely with GPs to assist in the management of chronic disease patients, conducting medication reviews and ensuring optimal drug therapy.
* Use the EMIS system effectively for patient record-keeping, prescription generation, and medication monitoring.
* Collaborate with the wider healthcare team to develop and implement treatment plans for complex or multi-morbid patients.
* Offer clinical expertise on the safe and appropriate use of medications, and provide education and support to patients on medication adherence.
* Provide a high standard of care, acting as an independent prescriber for appropriate conditions.
* Take an active role in audits, clinical governance, and quality improvement initiatives.
* Participate in team meetings, peer reviews, and CPD activities to ensure continued professional development.
Essential Qualifications & Experience:
* Qualified Independent Prescriber with active registration with the General Pharmaceutical Council (GPhC).
* Experience using the EMIS clinical system within a GP practice environment is essential.
* Previous experience working within a GP practice or primary care setting, with a sound understanding of the dynamics of a multidisciplinary healthcare team.
* Demonstrable experience of managing medications, prescribing, and conducting medication reviews in a primary care setting.
* Excellent communication skills, with the ability to work collaboratively within a team and deliver patient-centred care.
* Strong problem-solving skills and the ability to manage complex patient cases effectively.
* Registration with the GPhC and a valid Enhanced DBS check.
Desirable Skills:
* Experience in the management of long-term conditions such as diabetes, asthma, hypertension, etc.
* Knowledge of NHS guidelines and local formulary requirements.
* Previous involvement in audits and quality improvement initiatives.
