Cornwall has designed a collaborative to listen and respond to care homes which has been created to deliver safer, kinder, and enhanced person-centred care to people that live in care homes with improved equality and integration. It is part of a wider transformation program supported by Cornwall's Integrated Care System and funded through the Ageing Well Program.
The Clinical Specialist will work across Cornwall, being responsible for the delivery of a program of support and development aligned to care homes within each of the three Integrated Care Areas (Central, West, and North/East). They will develop an interface with care homes at its heart and collaborate with the Local Authority, Primary Care Networks, Community Services and wider health and social care partnerships.
This is a varied autonomous role with clinical assessment, research opportunities, quality improvement and training and education at its core. We are advocates for the residents and the care staff helping them to navigate through primary care.

Person Specification
Education / Qualifications and Relevant Experience
* Current professional registration - NMC, HCPC
* First Level degree in related subject or equivalent level of experience of working at a senior level in specialist area
* Relevant post registration experience in a senior role
* Evidence of competence in clinical assessment and clinical reasoning
* Experience of working as part of a multi-professional, multi-partner team
* Experience of teaching
* Leadership and management experience
* Coaching, Teaching or Mentoring qualification
Knowledge & Abilities
* Ability to work autonomously and as part of a team
* Ability to motivate self and others
* Organisation and negotiation skills
* Significant professional clinical experience, including experience of working with Older People Living with Frailty in Care Homes / Virtual Wards/ Enhancing Health in Care Home Programme
* Experience of working across the Integrated Care System footprint
* Experience of supporting transitions of care out of hospital
* Experience of Infection Control and Prevention outbreak management
* Experience of prioritising clinical tasks
* Evidence of managing and delivering complex multi-work stream programmes
* Evidence of ability to work under pressure under challenging circumstances
* Ability to support and influence change
* Teaching/assessment and presentation skills
