Job summary We are pleased to advise of an exciting opportunity to work within the Carers Information & Support Service for a fixed 12 month period. We are looking for an individual who can provide a high-quality social work service, providing care, carer assessment and caseload management support to unpaid carers and providing advice and support to other professionals, agencies, service users. Main duties of the job The role requires delivery of the statutory carers assessment, care and support plans that address people's hopes and aspirations, as well as their physical/environmental needs and support carers to remain in control. You will work jointly with other members of the CISS Team and partnership multi-disciplinary team to provide provisional packages of care and make relevant referrals to other stakeholders and Local Authorities.
