An exciting opportunity has arisen to manage The Urgent Community Response Team (UCRT) based here in South East Essex.
The UCRT respond to the crisis needs of patients in their own homes, including nursing and residential care homes, to avoid admission to hospital or new 24-hour care. The service has access to senior nurse assessors, independent non-medical prescribers, occupational therapists and healthcare support workers to support patients to remain in their usual place of residence during an acute episode of illness or social care crisis.
The post holder will be expected to be involved in the long term development and management of the Frailty Virtual Ward (FVW) as part of the virtual hospital.
Main duties of the job
The post holder will provide operational management responsibility for these areas providing effective leadership and hold a clinical leadership responsibility for the standards of clinical practice and services. This will include effective management of the multi-disciplinary team and financial budgets.
The Clinical Manager will be responsible for promoting excellence in order to deliver a high quality person-centred service, which is dynamic, proactive and responsive to health and social needs. They will deliver effective and safe services which support the key elements of clinical governance and focus on leading the clinical teams to introduce ways of continuous improvement in the quality of their services. The post holder will ensure that improvements in the planning, implementation and evaluation of care are based on user involvement and feedback from patient networks, their families and stakeholders.

Job responsibilities
1. Provide a visible, accessible and professional presence for staff, service users and their families.
2. Act as professional lead for the service utilising expert knowledge and skills.
3. Have overall responsibility for the safe and effective delivery of clinical services the post holder is responsible for.
4. Ensure care is person-centred and underpinned by evidence based practice and continual knowledge and skills of staff.
5. Ensure care is co-ordinated, patient focused and underpinned by evidence based practice.
6. Ensure both the overall and day to day skill mix is appropriate to ensure the best possible clinical care.
7. Ensure that patients privacy and dignity are protected and that patients are treated with respect at all stages of their patient journey.
8. Support systems for measuring and improving the quality of the patient experience within the service.
9. In collaboration with the Head of Intermediate Care and Head of Therapies, ensure there are systems in place to learn from all patient experience safety and outcomes information and develop systems of audit to ensure continuous improvement.
10. Ensure effective and confidential communication between medical, nursing and health care agencies on significant data governing patient care and treatment, whilst adhering to professional Codes of Conduct.
11. Undertake management and investigation of patient safety incidents in relation to nursing issues.
12. Manage complaints in line with agreed Care Unit processes and where required to resolve complex complaints, meet with complainants to address their concerns to ensure local resolution is achieved wherever possible.
13. Provide and receive a range of highly complex information and deal with sensitive issues, where there may be barriers to understanding and the audience could be hostile e.g. contentious issues, serious untoward incidents or serious complaints.
14. Use a high level of interpersonal and communication skills using sensitivity, tact and negotiation.
15. Ensure the safeguarding agenda is implemented with effective systems throughout the service including effective liaison with other agencies as required.
16. In collaboration with the senior management team, support the Care Units approach to improving compliance with standards, CQC registration and other regulatory requirements.
17. Work with the senior management team to ensure the delivery of the Care Unit and Corporate Objectives.
18. Support the Care Unit with the delivery of the CQUINs.
19. Be responsible for providing governance and assurance mechanisms for the services the post holder is responsible for, reporting to the Head of Intermediate Care or Head of Therapies.
20. As part of the Care Unit management team, manage the delegated budgets and ensure the financial targets for the team is met.
21. Comply with financial processes, standing financial instructions and deadlines.
22. Ensure relevant financial information is presented to staff and meetings, as appropriate.
23. Implement systems and controls for the effective recruitment, selection and retention of staff within the area of responsibility.
24. Ensure that the services meet responsibilities to provide clinical experience and mentor support for pre and post registration nurse and AHP education (when applicable), in line with internal and external standards.
25. Deputise for the Head of Intermediate Care when required.
26. Participate in the local Manager on Call rota for South East Essex.
27. Maintain own professional registration and practice requirements.
