Job summary Could you be the person to support excellent and timely discharge planning for patients in intermediate care bedded units. Based in Community Hospitals and Care Homes, our small and friendly Bedded Flow and Discharge Team support patients, their loved ones, therapy, social care, nursing and the wider multi disciplinary team to have excellent discharges. No two days are the same, whether you are supporting someone to arrange some shopping and go home independently, talking to families about finding a suitable care home, or joining up the dots to make a successful D2A happen, you'll be working alongside a supportive MDT to ensure our patients get the best discharge possible. You'll need a compassionate, organised and flexible approach, with excellent knowledge of community services and fantastic communication skills. You will need to be competent with a variety of IT, such as Microsoft Office; Teams and Rio. You will be based across 2 to 3 sites so the ability to travel within an agreed area is also essential. Main duties of the job To assist and facilitate patient flow through intermediate care bedded units and support patients to access a timely discharge process, from point of admission to point of discharge to the patient's own home or alternative destination.
