An exciting opportunity has arisen for a highly motivated and enthusiastic band 6 Nurse to join the hospital at home team in South Manchester.
The hospital at home team enables patients to receive safe, acute care, monitoring and treatment in their own homes.
We are an enthusiastic, motivated multi-disciplinary team working in a dynamic, exciting environment, providing an assessment, discharge and hospital avoidance service. Embracing these models, we will support the Emergency departments, AMU and Clinical decision units and work closely with our frailty teams.
Underpinning the purpose of hospital at home our established “Home First” approach supports patient’s personal choices, strengths, and ensures inclusion in every stage of their care. We adopt early mobilization which aims to reduce acquired hospital deconditioning, enabling a person to maintain as much of their independence as possible. We are passionate about continually developing our services to provide gold standard care for older people.
Main duties of the job
1. Undertake comprehensive health care needs assessment of patients, reassessing as appropriate.
2. Assess, plan, prioritise, implement and evaluate nursing interventions to meet patients’ needs.
3. Contribute to the organisation of multi-disciplinary, patient centred care.
4. Facilitate patients, relatives and carers to take an active role in care given, providing information as appropriate to facilitate effective discharge.

Person specification
Qualifications
* Degree/Diploma in Nursing
* NMC Registration
* Evidence of relevant post registration training / CPD
* Relevant Post graduate Training
Experience
* Community experience minimum of 2 years
* Broad experience as a Band 5 in core clinical areas, e.g. Experience in the care of neurological disorders, common orthopaedic, COPD and understanding of common mental health problems faced by older people.
* Experience of teaching others and mentoring students
* Experience of urgent assessments and developing and delivering care plans to meet these needs
* Clinical supervision and training
* Evidence of continuing professional development
* Experience of Rapid Response / Intermediate Care or similar
* Knowledge of speciality
* Evidence of involvement in policy and practice changes arising from audit
