Job Overview: We are seeking a skilled and motivated Band 6 Occupational Therapist to join our community health team. This role will focus on providing holistic and patient-centered care in various community settings, supporting individuals with complex rehabilitation needs to achieve independence and improve their quality of life. Key Responsibilities: Conduct assessments of patients in their home or community setting, developing personalized care and rehabilitation plans. Deliver high-quality interventions to support patients' mobility, daily living skills, and mental wellbeing. Coordinate with multidisciplinary teams, including physiotherapists, nurses, and social care professionals, to ensure comprehensive care. Support and educate patients, families, and caregivers on rehabilitation techniques, equipment use, and safety practices. Contribute to discharge planning, ensuring patients can safely transition back to their home environment with appropriate support. Requirements: HCPC registration as an Occupational Therapist. Experience in community healthcare or relevant clinical settings. Strong assessment, planning, and rehabilitation skills with a patient-centered approach. Excellent communication and teamwork abilities, especially in multi-agency collaboration. Ability to work independently and adapt to varied community environments.
