Dementia Intensive Support Team - Senior DIST Practitioner Band 6 (Mental Health Nurse/OT/Social Worker)
DISTe is a community team covering East Suffolk; driving is essential. We have an office base at Walker Close in Ipswich and often work from home. DISTe aims to prevent avoidable/inappropriate admissions to acute hospitals and to support with discharges. The team supports people with dementia or suspected dementia who are at risk of admissions caused by Biological and Psychological Symptoms of Dementia such as depression, anxiety, or challenging behaviour.
The service shall focus on:
* Maintaining independence for people with dementia
* Reducing the number of acute hospital admissions and readmissions
* Reducing the premature need for residential care in partnership with social care

Main duties of the job
Duties will include, but are not exclusive to:
* Assessments of People Living with Dementia or suspected Dementia
* Caseload holding
* Multi-agency working; including family, acute hospital, primary care sector, social care, care providers, and voluntary services
* Triage of referrals
* Working with Patients and Carers on a one-to-one basis
Supporting the Duty role within the Team.
The post holder will undertake the planning and delivery of direct patient care with appropriate supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment. Responsibilities include managing a caseload and the provision of Mental Health Care services to people with mental health care needs, supporting and advising other health care professionals, and participating in mental health promotion.

Qualification
* Nursing degree or equivalent
Experience
* Experience of working within a multidisciplinary team
* Familiar with risk assessment and its management
* Previous community experience
Knowledge
* Working knowledge/application of the Mental Health Act
* Knowledge of mental health needs for older people

* Ability to travel independently
* Willingness to undertake further training and development specific to role and needs
