This role involves working to support discharge from Alderney Hospital for patients who are over 65 years old and eligible for S117 aftercare under the Mental Health Act. You will be working closely with patients, their families and carers, and health colleagues to complete holistic Care Act assessments and Section 117 after-care assessments. You will be working closely with two wards within the hospital. An understanding and experience of multi-disciplinary working is essential for this role, as is experience of working with The Mental Health Act - particularly Section 117. About you : You will be a dynamic social worker with a strong value base and sense of person-centred practice who can use a strengths-based approach. You will be required to have previous experience of working with individuals who self-neglect and adult safeguarding. You will have excellent communication skills, enjoy working in a fast-paced environment, have a flexible approach and be able to work closely with a range of professionals.
