The Advanced Nurse Practitioner (ANP) at Chadderton MedicalPractice plays a vital role in delivering high-quality, patient-centred care.As a highly skilled and experienced healthcare professional, the ANP hasdemonstrated advanced clinical competency and expertise in diagnosing,treating, and managing a broad range of health conditions. Operatingautonomously, the ANP assesses patients, makes independent clinical decisions,and initiates appropriate interventions, including prescribing medications andordering diagnostic tests. ANPs at Chadderton Medical Practice work closelywith patients, developing long-term relationships built on trust andcollaboration. By offering expert care without the need for GP referral, theANP ensures that patients receive timely treatment, reducing delays andenhancing overall healthcare access. Main duties of the job
Develop and lead health promotion and wellbeing programmes.
Manage acute and chronic medical conditions, creating andevaluating treatment plans.
Provide advanced nursing care, wound care, and specialistclinics (, contraception, ear syringing, travel medicine).
Request and interpret pathology tests, and maintain accurateclinical records.
Lead chronic disease management, including maintainingdisease registers.
Prescribe medications, deliver immunisation programmes, andmanage mental health needs.
Ensure safeguarding, supervise staff, and contribute topractice targets.
Participate in clinical meetings and respond to clinicalemergencies.

Job responsibilities
The following are the core responsibilities of the AdvancedNurse Practitioner. There may be, on occasion, a requirement to carry out othertasks; this will be dependent upon factors such as workload and staffinglevels:
Develop, implement and embed health promotion and wellbeingprogrammes
Manage patients presenting with a range of acute and chronicmedical conditions, providing subject matter expert advice
Implement and evaluate individual specialised treatmentplans for chronic disease patients
Identify, manage and support patients at risk of developinglong-term conditions, preventing adverse effects to the patients health
Provide advanced, specialist nursing care to patients asrequired in accordance with clinical based evidence, NICE and the NSF
Request pathology services as necessary
Process and interpret pathology and other test results asrequired
Provide chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required
Maintain accurate clinical records
Ensure SNOMED CT codes are used effectively
Maintain chronic disease registers
Chaperone patients where necessary
Prioritise health issues and intervene appropriately
Support the team in dealing with clinical emergencies
Recognise, assess and refer patients presenting with mentalhealth needs
Implement vaccination programmes for adults and children
Support patients in the use of their prescribed medicines orover the counter medicines (within own scope of practice), reviewing annuallyas required
Contribute to practice targets (QOF, etc.), complying withlocal and regional guidance
Liaise with external services/agencies to ensure the patientis supported appropriately (vulnerable patients, etc.)
Delegate clinical responsibilities appropriately (ensuringsafe practice and that the task is within the scope of practice of theindividual)
Support the clinical team with all safeguarding matters inaccordance with local and national policies
Understand practice and local policies for substance abuseand addictive behaviour, referring patients appropriately
Deliver opportunistic health promotion where appropriate
Provide supervision and support to other members of staffsuch as Healthcare Professionals, Practice Nurses, Nurse Associates and NursePractitioners
Attend clinical, MDT and practice meetings
There may be, on occasion, a requirement to carry out othertasks. This will be dependent upon factors such as workload and staffing levels

Qualifications
Essential
1. Registered Nurse with Nursing and Midwifery Council
2. Master's Degree required for qualification post.
3. Post Graduate diploma or degree for Advanced Practice Qualification.
4. Qualified Independent Nurse Prescriber on the NMC Register.
5. Meets NMC re-validation requirements in accordance with the NMC Re-validation booklet.
6. Minor illness qualification.
