Assistant to the Care Navigation Team Manager
We are looking for a part-time assistant to our Care Navigation Team Manager with experience working in a General Practice and using the clinical system Sytmone.
The role is an essential part of our friendly and experienced care navigation team providing the first point of contact for our patients.
This is a part-time role over 4 or 5 days a week. Occasional Saturday morning and evening (no later than 8pm) working may be required in accordance with the practice enhanced access rota. One day per week is until 6.30pm.
Main duties of the job
The postholder will assist in supervising the care navigation team and offer general assistance to the Care Navigation Team Manager. You will be responsible for assisting with the day-to-day supervision of reception and care navigation duties and staff, ensuring that the correct procedures and protocols are being followed, reporting to the Care Navigation Team Manager and Business Management Assistant.
You will act as a role model to the care navigation team, supporting, motivating, and promoting staff relations, as well as undertaking a variety of administrative duties and answering calls to assist in the smooth running of the practice.

We are a friendly team of 6 partners and 3 salaried GPs with a supportive and experienced management team. In addition, we have a large supportive and experienced administrative team covering secretarial, document processing, IT support and long-term condition recalls.
We have a large and experienced nursing team qualified in multiple areas with a supportive HCA team and phlebotomist working alongside them.
Actively engaged with our local PCN, we utilise the services of a very experienced Health and Wellbeing team that look after our frailty patients. We also have FCPs and Social Prescribers that work in our practice.
We are a training practice having registrars and medical students in all stages of their training. We actively encourage all our staff to develop professionally.
Job responsibilities
Job responsibilities:
1. Ensure processes are carried out in accordance with agreed procedures, protocols and time-scales, reporting to the Management team as necessary.
2. Supervise the care navigation team in the absence of the Care Navigation Team Manager.
3. Offer general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
4. Facilitate effective communication between patients, members of the primary healthcare team, secondary care and other associated healthcare agencies.
5. Act as a role model to the care navigation team, supporting, motivating and promoting staff relations.
6. Undertake a variety of administrative duties to assist in the smooth running of the practice.
7. Ensure adequate staffing levels: organise Holiday/Sick rotas in line with agreed policy liaising with the Care Navigation Team Manager.
8. Continually assess and evaluate systems recommending changes and improvements to the Care Navigation Team Manager as appropriate.
9. Computer data entry/data allocation and collation; processing and recording information in accordance with practice procedures.
10. Process prescriptions as required and according to Practice protocols.
11. Assist with induction and training of new care navigation staff.
12. Assist with patient complaints.
13. Participate in audit where appropriate.
14. Apply practice policies, standards and guidance.
15. Oversee the alteration/cancellation of surgeries and make alternative arrangements in an emergency.
16. Ensure the NHS111 and out of hours reports are processed in a timely manner.
17. Oversee and delegate daily tasks in the absence of the care navigation team manager.
18. Have a full understanding of the appointment system within the practice.
19. You may, on occasion, be required to work Saturdays and evenings (no later than 8pm) in accordance with our Enhanced Access Rota.

Qualifications
* GCSE or NVQ Level 3 or equivalent experience or above
* NVQ in business administration
