We are looking for regular experienced GPs to join our integratedteam working.
The role will be based at Nelson House in Hereford.
This role would ideally suit GPs who would welcome the time tounpick complex medical histories and support proactive care planning; thosewith an interest in frailty; and those who enjoy supporting the learning anddevelopment of advanced practice colleagues.
Main duties of the job
The GP will provide telephone and video support to the community ACPs, nurses and therapists assessing patients in the community. The GP will support patients who are managed in a GP-run virtual ward. The GP will also support paramedics and the ambulance service for patients who do not need conveyance to hospital but may still need some medical intervention.
