Morden Primary Care Network is looking to recruit a Care Coordinator to join one of our practice teams. Morden PCN encompasses Morden Hall Medical Centre, Central Medical Centre, Stonecot Surgery, and Ravensbury Park Medical Practice, covering a total registered list of around 39,000 patients. The primary practice base will be at Central Road Medical Practice. The PCN practices strive to provide high-quality patient care and have a strong base of education and training.
Main duties of the job
The main role of the successful applicant will be to proactively communicate information between patients, doctors, and professionals. This will include assisting the clinical teams with administrative functions within the practices. There will be peer-learning opportunities for the care coordinators across the PCN and also training and development opportunities. This role requires excellent communication, IT, and organisational skills, including the ability to prioritise, multitask, and work to deadlines. The successful applicant will be a key member of the base practice teams and will need to work to a high standard. Experience of working in a Primary Care setting would be desirable.

RESPONSIBLE TO:

JOB PURPOSE:
To work across the 4 practices that make up Morden PCN.
1. Organise and work with video consultation groups.
2. Monitor data submitted by patients online through the E-hub and ensure it is dealt with appropriately.
3. Proactively communicate information between patients, doctors, and professionals.
4. Assist the clinical teams with administrative functions within the practices.
5. Assist the doctors and other clinicians in the provision of a high-quality service across the practices.
6. Administer information received via the central e-hub for the PCN and ensure it is dealt with appropriately and efficiently.
7. Organise and facilitate video group consultations (training provided).
8. Seek patient information and results from hospitals and other organisations.
9. Answer general enquiries, making new and follow-up appointments.
10. Provide results to patients under clinical direction.
11. Contact hospitals to follow up on referrals.
12. Liaise with hospitals for updates on patients or advise of any relevant changes in patients' conditions as directed by clinicians.
13. Complete and submit quality alerts as appropriate.
14. Deal with requests for information (e.g., Merton MASH).
15. Organise and attend MDT meetings.
16. Take notes at MDT meetings and prepare associated minutes.
17. Update Coordinate My Care register.
18. Book follow-up appointments for patients requested by clinicians.
19. Support the reception team in dealing with transport requests.
20. Review, filter, and flag information on Docman requiring urgent action to clinicians.
21. Contact and chase patients as requested by the clinicians.
22. Proactively ensure that booked appointments are appropriate and with the correct clinician.
23. Oversee reports and prompt doctors when urgent. Prepare eGPRs where necessary.
24. Look up results on the EPR system or chase with hospital/lab.
25. Assist clinicians with care plans. Coordinate and action admin aspects of care plans.
26. Undertake quality improvement and change in practice. Assist with audits as required.
27. Understand the importance of medical terminology, data, and coding.
28. Maintain record keeping with appropriate coding.
29. Understand vulnerable groups and long-term illness. Use patient information resources.
30. Work as a positive and active member within the Practice and PCN teams.
31. Participate in an annual appraisal at which your performance will be reviewed, and your training needs discussed.
32. Attend relevant meetings as required, including staff meetings. You may be requested to attend other meetings at the request of the medical or management team.
33. Support, supervise, and mentor others.
34. Participate in peer learning with other Care Coordinators within Morden PCN.

Qualifications
* Experience of working in customer care.
* Experience of working in an office-based environment.
* Experience of Microsoft Office applications.
* Good communication skills, both verbal and written.
* Ability to work alone or as part of a team.
* Ability to work to and meet deadlines.
* Experience of working in a GP practice.
* Knowledge of clinical IT systems (EMIS, Docman, AccuRx, etc.).

Salary: £23,400 a year including Outer London weighting.
