We are seeking an organised and empathetic Admission & Discharge Co-Ordinator to ensure seamless transitions for our patients, providing crucial support in managing the admission and discharge process within our facility.
Position Overview
The Admission & Discharge Co-Ordinator plays a vital role in coordinating patient transitions, working closely with clinical teams and families to facilitate smooth admissions and discharges. This position requires meticulous attention to detail and a strong commitment to patient care, ensuring that all processes are optimally managed and documented.
Key Responsibilities
1. Coordinate with medical staff to schedule and manage patient admissions and discharges.
2. Maintain accurate records of patient data and ensure compliance with medical regulations.
3. Communicate effectively with patients and families to explain procedures and provide support.
4. Collaborate with multidisciplinary teams to optimise patient care plans and transitions.
5. Identify and address any issues or delays in the admission and discharge process.
6. Ensure all legal and ethical standards are met during the transfer process.
Required Skills and Qualifications
1. Previous experience in a healthcare administrative role or similar capacity.
2. Exceptional organisational and multitasking skills.
3. Strong communication skills to liaise with healthcare professionals and families effectively.
4. Proficiency in using healthcare management systems and software.
5. Empathy and understanding of the challenges faced by patients and their families.
Salary: £24,301.27 to £29,408.76 per year (depending on experience)
Contract: Permanent
Hours: Full Time, 37.5 hours per week
Location: Basildon Hospice and Thurrock Hospice
Additional Requirements
1. Ability to work flexible hours, including evenings and weekends, as needed.
2. Right to work in the UK; sponsorship is not available.
