Key Duties and Responsibilities To co-work with the Care Co-Ordinator/case manager and within the health, education and social care systems, delivering a collaborative person-centred approach to progress a timely discharge and avoid admission. Utilise their professional knowledge and experience to inform their clinical and commissioning oversight of the individual to be instrumental in system navigation, to ensure the person remains at the centre of their care and treatment. To chair and attend Care (Education) and treatment reviews C(E)TRs for people in in-patient services. Chair preadmission, inpatient and post discharge C(E)TRs supporting the admission avoidance pathways that are well established and identify and escalate any gaps in the system that lead to admission or delay discharges. To ensure the person is not being cared for in a restrictive setting for longer than they are clinically assessed to be. To ensure a least restrictive setting as close to home as possible is being considered. To ensure that the Quality assurance standards are met, and service providers implement outcome recovery pathways for individuals to enable the person to develop new skills that support them to live in the community, promoting a timely discharge. To maintain reporting systems which monitor the discharge pathways with providers. Review and monitor the recommendations of C(E)TRs and/or recovery outcomes for individual commissioned on section 117 after care when appropriate. Work within the admission avoidance pathways to support those being discharged back into the community to ensure that the assessed needs are being meet within their placement with particular regard to race, religion, culture, ethnicity, disability or sexual orientation. To attend and be an active member of MDT meetings bring services together to support timely discharge. Coordinate the development of the individual specification required to inform timely discharge. Lead liaison with LA commissioners to support the development of suitable, sustainable accommodation for people returning to H & W following admission. To review and monitor the PBS and risk management plans to ensure they meet assessed needs and inform commissioning intentions appropriately for the person. Provide advice and direction as required to inpatient and community teams bringing services together to meet needs holistically. To act as a senior clinical member of the NHSICB commissioning teams and play a lead role in safeguarding vulnerable adults by leading Adult Protection Investigations where appropriate. Complete Quality Assurance visits. Manage and respond to queries from GPs, Consultants, Clinicians and patients/carers advising on placement quality and outcome.
