We are looking for an inspiring individual to join our team at The Lewes Intermediate Care Service as a Ward Sister/Charge Nurse Band 6.
Lewes Intermediate Care is a 26 bedded unit. The unit is very friendly and welcoming with a great multidisciplinary team who work together with patients and their relatives/carers to support rehabilitations and safe discharges to the most appropriate location for them.
Please note that the unit runs a 24 hour service so internal rotation is required.
Main duties of the job
The role of the ward sister/charge nurse is varied and a great development opportunity within a stand alone unit. You will support the Ward Manager with staff management, incident management, patient flow and roster management. You will provide clinical expertise and visible leadership whilst helping to create a positive patient experience.
We welcome applications from already experienced Band 6's or experienced Band 5's seeking development.
Job responsibilities
1. Assess, plan, implement and evaluate patient care with MDT colleagues to help ensure patients meet their rehabilitation potential.
2. Empower patients to work as partners in their care, respecting individual choice and difference and encouraging self-care where able.
3. Identify significant change in patients' physical, psychological and social status acting on findings, seeking guidance and/or reporting to the appropriate member of the multi-disciplinary team as well as the ward manager.
Person Specification
Qualifications/Professional registration
* Adult registered nurse qualification - NMC
* Evidence of a range of clinical competencies
* Evidence of post-registration development
Experience
* Years experience at staff nurse level
* Relevant experience in an acute trust setting
* Experience in complex discharge planning and transfers of care
* Recent experience in an in-patient rehabilitation/elderly care setting
Skills and Knowledge
* Able to demonstrate the application of research based practice
* Knowledge of current local policies informing community health and social care services
* Able to promote independence and choice in undertaking patient care
* IT literate
* Clear knowledge of clinical governance, professional codes of conduct and Trust policies
* Clinical Supervision skills
* Advanced communication and negotiation skills
Other requirements
* Demonstrate ability to reflect and learn from situations with a commitment to professional learning and development
* Clear examples of co-operative team working
* Identifies difficulties as challenges and works with others to identify solutions
