PCN Advanced Clinical Practitioner Nurse
As an advanced practitioner, this post holder will work collaboratively across the PCN to play a crucial role in clinical governance, building capacity and capability, and providing resolution and critical thinking to some challenging patient diagnoses or issues.
Whilst operating at an advanced practitioner level, you will drive forward service improvements and clinical governance alongside seeing patients face to face, doing care homes ward rounds, and making home visits. The PCN will benefit from your level of knowledge, skills, experience, and leadership to build capacity and capability across the network.
Main duties of the job
1. To assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines, and local care pathways) or make necessary referrals to other members of the primary care team.
2. To manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
3. To use complex decision making to inform the diagnosis, investigation, and complete management of episodes of care within a broad scope of practice.
4. To actively take a personalised care approach and population-centered care approach to enable shared decision making with the presenting person.
5. To complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example, first contact practitioners and the personalised care roles.
6. To provide specialised clinical knowledge to assess patients, order and interpret diagnostics, and initiate treatment.
7. To provide skilled team leadership to clinicians within the wider multidisciplinary team.
8. To provide a high level of clinical expertise.
9. The post holder is an autonomous practitioner who will manage a caseload of patients, using evidence-based practice/patient-focused principles to assess, plan, implement, and evaluate interventions.
About us
GP practices have been working together in our area for years and formed Bridgedale Primary Care Network (PCN). Bridgedale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre, and the Ryan Medical Centre. We serve a community of around 30,000 people. We are small enough to provide the personal care valued by our people and large enough to have an impact and economies of scale through better collaboration.
As a Primary Care Network, we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024.
Job responsibilities
Key duties and responsibilities
1. 10. The post holder is an autonomous practitioner who will manage a caseload of patients, using evidence-based practice/patient-focused principles to assess, plan, implement, and evaluate interventions.
11. To ensure Shared Decision Making (SDM) is embedded within the service and evidenced.
12. To communicate appropriate complex information effectively to patients and/or carers where there may be barriers to understanding.
13. To inform them of the benefits and risks of suggested treatment options and to undertake this with groups or individuals.
14. To demonstrate sound reasoning of clinical governance, clinical risk, and to demonstrate the use of treatment plans with an evidence-based link.
15. To use advanced clinical reasoning skills enabling formal supervision, specialist support, and advice for the AARS roles on a regular basis and be available to advise other staff on the management of complex patients as appropriate, in areas of specialist expertise.
Building capacity through effective leadership
1. To provide clinical leadership to the AARS team, provide 1:1 and carry out appraisal and training plans when needed.

1. Act on own initiative in prioritising and managing workload, delegating tasks appropriately and referring to MDT Members in respect of clinical, managerial, and administrative areas, as necessary.
2. Enrolled into review and appraisal systems within the PCN.

3. Undertake any additional duties appropriate to the post as requested by the Partners/Practice Manager/PCN Manager and Clinical Director.
Respect for Patient Confidentiality
In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
The post-holder should always respect patient confidentiality and not divulge patient information unless sanctioned by the requirements of the role.
Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the Practice/PCN policies and procedures relating to confidentiality and the protection of personal and sensitive data.
The post-holder will comply with the Data Protection Act (1984) and the Access to Health Records Act (1990).

Safeguarding Children and Vulnerable adults
The PCN is committed to safeguarding children, young people, and vulnerable adults within its care. As an employee, you are accountable to ensure that you know how to respond when you are concerned for the safety of a child, young person, or vulnerable adult. The PCN will support you in this process by providing training, support, and advice. There is a safeguarding team for children and young people and leads for vulnerable adults, who can be contacted for guidance in each practice who provide safeguarding supervision. For children, you should be aware of your responsibilities detailed in the 4 Local Safeguarding Children Boards Child Protection Procedures and for vulnerable adults in the Safeguarding Adults Policy.
All employees have a duty to safeguard and promote the welfare of children and adults and are required to act in such a way that always safeguards their health and wellbeing. Familiarisation with and adherence to national and local safeguarding adults and children's policies is an essential requirement upon all employees.
Staff are also required to participate in related mandatory/statutory training and to ensure that they undertake this to a level appropriate to their role.
Special Working Conditions
The post-holder is required to travel independently between practice sites (and other locations such as the vaccination centres, care homes, and home visits), and to attend meetings etc. hosted by other agencies.
The post-holder will have contact with body fluids i.e., wound exudates while in clinical practice.
To be prepared to work flexibly, including evenings, bank holidays, and weekends if required.

* Have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centre's ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice.
* Registered clinical practitioner with their relevant professional body.
