37.5 hours per week. This exciting new role is part of our Neurodiversity Transformation Programme and is building on the successful LDAP (Learning Disability and Autism Pathway) Keyworker pilot that was designed in partnership with NHS England. The Neurodiversity Navigator role will be to support the child, young person and their families who are awaiting assessment for a diagnosis. They will focus on the young person's needs, supporting the family to navigate the system and access local support whilst waiting for their assessment.
We will continue to deliver a new way of supporting Children, Young People (CYP) and their family/carers that are having difficulties with any neurodiverse presentations. This role will enable CYP and their family/carers to get the right support at the right time and ensure local systems are responsive to meeting their needs in a holistic and joined-up way.
We are looking for enthusiastic and proactive candidates who want to make a difference to the lives of Children and Young People in Devon. We are looking to recruit multiple positions to work across education, health, social care and local communities to enable whole systems to change and embed principles of transforming care and SEND reforms to improve the experience of CYP and families/carers.
These roles are being hosted by LWSW but will be locality based across the Devon footprint and posts will be working as part of the local multi-agency response in Devon, Torbay and Plymouth.
Main duties of the job
We are recruiting a team of Neurodiversity Navigators that will provide support across the Devon County footprint. You will be based in locations across the county to provide local support for our families in an attempt to reach out to as many as possible.
* Support with navigating the complex networks of health, education, social care, community and voluntary services
* Informed advice
* Connecting people to community groups, parent/carer forums and statutory services for practical and emotional support
* Support completing forms
* Targeted support with simple personalised support plans
* Advocate when required but not in place of other advocacy services

Job responsibilities
The principal purpose of the job is to improve the health & wellbeing of CYP and their families/carers with a range of possible neurodiverse difficulties.
* Active involvement in the delivery of high quality, safe and compassionate service as an effective member of the team
* Implementing specific assessment, review and care packages to individuals and groups within standard protocols, and integrated care pathways
* Providing evidence-based advice and consultancy to young people, family members/carers within standard protocols
* Organising and managing a defined caseload under appropriate guidance and work as lone workers where appropriate
* The post holder will be accountable for their own actions within the delivery of this care. Seeking advice and expertise of a registered practitioner when required through the use of regular managerial and clinical supervision in accordance with Livewell policy.
Person Specification
Experience
* Previous experience working in a range of different health and/or care settings.
* Experience of working autonomously and using own initiative.
* Working with young people with challenging behaviour and complex health needs.
* Experience that evidences an ability to embrace diversity and work in a way that promotes anti-discriminatory practice.
* Experience of collaborative working in community settings, with professionals across a range of agencies in health, social care, and education.
* Experience of safeguarding children and ensuring the safety of young people in partnership with other agencies as appropriate.
Qualifications
* Foundation Degree in Health and Social Care or equivalent qualification in related subject.
* GCSE in Maths and English (Grade C/5 or above) OR Level 2 Key Skills / Functional Skills in Maths and English.
* Have a Care Certificate or willing to work towards achieving this certificate supported by Livewell.
* Evidence of additional relevant health training. Experience of working with young people. Social Care, Learning disabilities, Speech and language therapy and Psychology graduates seeking relevant professional experience.
Knowledge
* Knowledge of a range of Neurodiverse presentations.
* An understanding of physical health problems which can impact on a person's mental health and monitoring of the same.
* Knowledge of child protection and safeguarding procedures.
* Basic IT skills.
* Understanding of relevant legislation pertaining to children's physical and mental health.
* Knowledge of child development.
* Aware of the Code of conduct for Health Care Support Workers /Adult Social Care Workers in England.
Skills
* Excellent verbal, nonverbal and written communication skills.
