The Practice Nurse, in conjunction with the GPs, undertakes a wide range of assessments, screenings, and care for patients. They will provide a professional service to patients, doctors, staff, health service professionals, and others at all times. The management of patients with chronic diseases will form a large part of the workload. Practice Nurses are also required to undertake general treatment room duties including dressings, wound care, child/adult immunisations, venepuncture, and general health screening and health promotion advice.
Main duties of the job
1. Assist GPs to deliver effective management of patients with chronic conditions.
2. Assist GPs to undertake procedures where required.
3. Undertake opportunistic screening when clinically required and perform venepuncture and electrocardiography when required.
4. Undertake basic wound care and perform cervical screening and promote breast awareness.
5. Administer adult and paediatric injections under a Patient Group Direction, ensuring safe storage and disposal of vaccinations and drugs. Update relevant paperwork as necessary.

Fairfield Surgery is situated within the Port Talbot Resource Centre, with a vision to expand the workforce team. Our clinical team consists of two GP partners alongside three long-term GP locums, one Practice Nurse, a Health Care Worker, a Practice Pharmacist, and a Paramedic with an excellent supportive admin team.
The post requires an enthusiastic, highly motivated Practice Nurse to work within a busy but friendly practice. Experience of working in a primary care setting is desirable but not essential. We are a dedicated practice that delivers a high standard of care to a population of 8,200 patients in the Port Talbot area. The surgery is supported by an excellent clinical and administrative team.
The successful candidate will be required to undertake a full range of general Practice Nurse duties in conjunction with the GP, or independently, as appropriate, and according to Practice policy and protocols. Also, lead on Chronic disease clinics.
Job responsibilities
1. Provide nursing services to the patients accessing the practice.
2. Delivery of evidence-based practice for patients presenting with chronic conditions, and the provision of preventative health care to the practice population.
3. Work as an autonomous nurse within current competencies, demonstrating critical thinking and skills in clinical decision-making to support the management of patients.
4. Support new models of care within the managed practice, including telephone first concepts and alternative workforce roles.
5. Deputise for nursing colleagues during any periods of absence.
6. Support the Practice Nurse in delivering and supporting effective care to patients within the practice.
7. Work closely with the wider multidisciplinary team to ensure safe effective delivery of health care, including mental health and public health services.
8. Represent the practice viewpoint at operational and strategic meetings when necessary.
9. Act as an accountable role model and clinical expert for healthcare.
10. Promote and use the audit cycle within clinical practice.
11. Support the development and implementation of Healthcare Policies and procedures.
12. Monitor and evaluate the performance of team members and ensure adherence to practice Policies and procedures including sickness management and staff annual appraisals.
13. Supervise HCSWs and unregistered staff, including responsibility to assess staff competencies prior to nurse delegation.
14. Work in conjunction with other disciplines in the assessment, planning, implementation, and evaluation of healthcare service provision to ensure a seamless patient pathway.

* Registered Nurse: first level, on the NMC register.
