We have an exciting opportunity for a social worker to be part of a local clinical rehab team which is a joint initiative between ICB, MPFT and Telford & Wrekin Local Authority, with a focus on delivering rehab within our own community.
We are a small, dynamic, diverse and dedicated team that covers the whole of the Telford Borough. Our Council is proud of the fact that we constantly strive to deliver for local people and this is evidenced by our recent CQC rating of Good.
We are passionate in supporting people in their recovery journey to positive mental health and wellbeing. With an inclusive focus, incorporating their families, carers, and communities, to enable the person to live fulfilling and independent lives.
We work closely with the voluntary sector, Community Mental Health Services, Mental Health hospitals, Forensic services, housing and other relevant services in the community to support adults during their mental health journey. As a team, we have a presence in the community in the form of calm cafes, which we run jointly with Telford MIND and A Better Tomorrow to support people who are experiencing mental health crisis and comorbid addiction challenges.
The team has a strong connection with the Mental Health Alliance which is a collaboration of voluntary sector, statutory and NHS services adopting a Multi-disciplinary Approach which allows sharing of information to support individuals and communities to promote positive outcomes.
