Job overview
An exciting opportunity has arisen for a triage clinician to join our Neighbourhood Mental Health Team (NMHT) in South Birmingham. The main duties of the role will include triage, assessment, and brief interventions for patients with mental health difficulties being referred from primary care. Our community mental health services are well-established teams, as part of the BSOL community transformation programme. We are looking to increase our Neighbourhood Mental Health Team with this role, focusing on supporting GP colleagues with initial triage and assessment of mental health difficulties. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care.
You will be responsible for providing mental health triage, assessment, and outcomes-based intervention for service users within the Neighbourhood Mental Health Team. You will be responsible for triaging new referrals, identifying need, and determining the best course of advice, support, or intervention. You will also promote psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
Main duties of the job
The post holder:
1. Will be responsible for supporting the daily triage and assessment of new referrals.
2. Will consolidate historical information for new referrals into the Neighbourhood Mental Health Team and support the daily triage meeting.
3. Will be responsible for the assessment planning, implementation, and evaluation of care within agreed parameters, and have a working knowledge of local and national policies and legislation that govern current service provision and audit.
4. Will ensure effective communication between pathways exists.
5. Will promote equality, diversity, and rights in accordance with good practice and legislation.
6. Will identify and contribute to the training needs necessary in providing quality evidence-based care as appropriate.
7. Support colleagues in the effective assessment and delivery of clinical service for individuals within the assigned caseload.
Person specification
Essential criteria
* Registered Mental Health Nurse/OT/Social Worker/Psychological Therapist/Psychological Practitioner
* Relevant post-registration experience in mental health
Desirable criteria
* Comprehensive assessment skills and risk management skills.
