We are looking to recruit an enthusiastic, well-motivated team member to work within the discharge and patient flow team to facilitate safe and effective discharge from a ward area. Must have excellent communication skills, willing to learn new skills, have some understanding of safe discharge, and be able to liaise with all multidisciplinary teams.
Main duties of the job
The role of the discharge coordinator is to coordinate and facilitate safe and effective discharge, working alongside ward staff, therapy teams, discharge hub, and case managers.
Gathering patient social information on admission, referring to discharge hub in a timely manner when patients are medically stable for discharge.
Liaising with family/friend carers to discuss any concerns they may have for discharge.
The successful candidate will raise discharge concerns to the right teams as concerns arise. Use clinical systems, i.e., ICE, Care flow connect, careflow live, Pro-track.
You will have a full understanding of all the discharge pathways and the COVID policy for patients returning to care homes, ensuring swabs are in date and the discharge hub completes the correct risk assessment.
You will gather accurate information for the site matron of expected discharges on the ward daily and attend board rounds daily.
Excellent communication skills are required for this role.
