Job summary We are looking for a forward thinking, conscientious and compassionate Nurse to join our nursing team. You will be registered with NMC and may be newly qualified or already have experience of working within a GP Practice or PCN setting. There is the opportunity to develop within the role and learn new skills. Main duties of the job Scope & main responsibilities: a. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice. To develop, implement and embed health promotion and wellbeing programmes b. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing c. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health d. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF e. Work within own competence and professional code of conduct as regulated by the NMC f. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. g. We provide GMS to a registered list of 13,600 patients.
