We have the exciting opportunity for a Community Support Worker to join our Newcastle Housing with Care and Support team This is a permanent, part-time role contracted at 22.5 hours per week. Shifts are 3 x 7.5 hour shifts per week (including some weekends and unsocial hours, additional on call payment and enhanced pay for unsocial hours/weekend) It is an essential requirement of this role that you are a driver with access to your own car. As a Community Support Worker, you will: Support tenants to maintain their tenancy within the community and support planning and evaluation under the supervision of a Registered Mental Health Nurse for a small caseload of service users to provide ongoing assessment and evaluation. Demonstrate and promote daily living skills, home management skills and socialisation and form and maintain relationships with tenants’ friends and relatives to gain insight and effectively address service users’ needs. Motivate and promote participation in social/recreational/occupational activities and promote a healthy balance between rest and activity to improve mental health and wellbeing. Provide assistance in response to emergency situations in a first aid capacity and contribute to the creation of safe and homely environments to keep service users safe. Collaborate with external agencies and attend and contribute to Support Needs Evaluation and Care Co-ordination meetings for tenants as their co-worker to support informed decision-making by tenants. Conduct assessments on risk factors for tenants’ ‘daily activities’ and mental health, then liaise with the nurse on call, as necessary to ensure needs are met. Guide and advise tenants’ of other services/support to develop their skills to more independent living environments and promote tenants’ participation in their own support programmes, encouraging attendance at service-user forums to encourage independence About you We would love to hear from you if you possess the following: Demonstrable experience of providing a regular and effective service Demonstrable experience of working 1:1 with service users people with complex needs Demonstrable experience of working within health, social care • Understanding of recovery in the context of mental health Understanding of the effects of stigma and discrimination Understanding of confidentiality and ‘General Data Protection Regulations’ Understanding of the health and social care landscape What we offer in return We are proud to have been recognised and certified as a Great Place to Work, which speaks volumes on how much we value our staff members.
