As the largest local authority in Wales we support nearly individuals to live well, offering them a person-centred support structure to ensure they can meet their own needs and outcomes.
Adult Community Services is our largest team in Cardiff supporting anyone over the age of with physical disabilities and older people. The teams are made up of community and hospital based Social Workers who offer services for individuals and families from the first time they need support from Adult Social Services throughout their journey with us. Working alongside a wider multi-disciplinary team, including Occupational Therapy teams, Social Work Assistants, Day Centre teams and Health colleagues we find solutions to best support individuals to achieve their outcomes. We have contractual relationships with a wide range of home care providers, offering care and support throughout Cardiff and residential care homes, ensuring that all options can be available to best meet an individual’s needs.
Within the Adult Community Services there are a number of teams who ensure that individuals are supported in the right place at the right time:

An exciting opportunity has arisen to join Cardiff’s Adult Services Hospital Social Work Team.
Wales’s vibrant capital city offers you the opportunity to work within the largest and most diverse community in Wales. Our Adult Services teams support over individuals and offer a wide range of professional opportunities for both newly qualified and experienced Social Workers.
You will join a team that specialises in supporting older people and individuals with physical disabilities and which is committed to strengths based, preventative social work, supporting people to live fulfilled lives and to remain as independent as possible.
You will work across three hospital sites in Cardiff: University Hospital of Wales, St David’s Hospital and University Hospital Llandough.
You will be required to work as part of an Intergrated Discharge Service, which is multidisciplinary, working alongside health colleagues and therapists to ensure safe and timely discharges of patients back into the community, supporting them in a strengths based and person centred way to achieve their desired outcomes.

What We Are Looking For From You
We are looking for qualified Social Workers early into your career and looking to gain further experience within Cardiff Council, as well as Social Workers with experience already behind them. As the successful candidate you will be able to work independently and manage your time. You will be able to show you have excellent skills in assessment and the ability to write concise records and reports. You will be a good team player, have excellent communication, organisation skills and IT skills.
You will be supported by an experienced social worker who will offer mentorship and guidance on your cases and will support your career development within the team.
You will have a commitment to supporting older people and those with physical disabilities and to working in a strength-based, preventative way, providing the support needed to enable them to achieve what matters to them.
You will have a strong and demonstrable commitment to strength-based approaches, a dedication to working in an outcome-focussed way and a keen interest in your on-going professional development, striving for excellence.

A degree in Social Work or equivalent Social Work qualification and registration with Social Care Wales are essential for this role.
