Job Summary
ICT - Community Healthcare
To provide clinical interventions to designated patients, both physio and occupational therapy, within scope of practice, on behalf of qualified practitioners within the multidisciplinary team. To supervise and delegate work to more junior staff within the team. To be involved in service development to include delivering training as appropriate to other members of the team.
Main duties of the job
Key Responsibilities
1. The post holder is responsible for actively participating as part of the Multi-disciplinary Team (MDT) to carry out specific planned treatments/interventions within tested areas of competency which help to improve client's quality of life/functional abilities/wellbeing so enabling them to remain within their own environment, preventing admission to hospital or other residential accommodation as well as facilitating early discharge from hospital.
2. To review treatment care plans making necessary modifications prior to reporting back to a qualified staff member.
3. To assess for and provide equipment; assess and process requests for minor adaptations.
4. To provide support and training to other staff within the team.
5. To proactively monitor and review the non-qualified caseload and actively monitor patients escalating both when deteriorating and when requiring transition from the service.
Car ownership and the ability to drive is a requirement of this role as this is a community post.

Knowledge And Experience Essential
1. Completion of NVQ L4 or Foundation Degree level or equivalent in health related subject
2. Grade C or above GCSEs or equivalent in English and Maths
3. Experience of working with physically impaired people in a therapeutic environment
Desirable
1. Working knowledge of medical conditions commonly seen within Health Care area e.g. Stroke, Parkinson's Disease, Orthopaedic conditions, Diabetes, amputations, Multiple Sclerosis etc and how this impacts on the patient
Skills and Abilities Essential
1. Evidence of extended competencies especially with patients in the community
2. Experience of working within a pressured environment
3. Good verbal and written communication
